Changing PTDE Instructor

If you purchased a PTDE Teaching Guide and need to change the Instructor you can do so without having to repurchase the Teaching Guide.

Fill out this form or email the TDLR including the following:

1. the receipt number and/or the student’s full name and date of birth;
2. the new instructor’s name; and
3. how you would like to receive the new guide (mail or email).

It may help if you include the original email address the packet was ordered under.
